■  Registered Specialists and Therapists. Includes visits to Chiropractors, Acupuncturists, Osteopaths, Podiatrists, Naturopaths,
Chiropodists, Registered Massage Therapists, Physiotherapists, Psychologists and Speech Therapists.
■  Homecare and Nursing. Medical care in your home from a Registered Nurse, Registered Practical Nurse, Certified Home Support Worker, Occupational Therapist, Registered Dietitian, Registered Nursing Assistant or healthcare aide.
■  Prosthetic Appliances and Durable Medical Equipment. Surgical bandages and dressings, purchase or rental of equipment
such as crutches, non-electric wheelchairs and hospital beds, oxygen and more. Also includes prosthetic appliances such as artificial limbs, eyes, splints, casts and breast prostheses.
■  Accidental Dental. Payment towards dental treatment when it is required due to an accidental blow to the head or mouth.

■  Lifeline® Personal Response Service. Provides 24-hour monitoring service for people coping with medical problems at home and

■  World-Class Second Opinions. Offers evaluation of medical records upon diagnosis of serious illness or injury.
■  Travel*. Covers emergency hospital/medical expenses while travelling outside your home province.
■ Accidental Death and Dismemberment. Payment for accidental death or dismemberment directly resulting from an accident within one year of the date of the accident.
■  Survivor Benefit. Provides for coverage to be continued for one year following the death of an adult policyholder.

Apply today
Contact your advisor today so your coverage can begin as early as the first of the month following your application. Have this information in hand:
■  The provincial health card numbers of everyone applying for coverage
■  Your Physician’s telephone number
■  The name of any prescription medication you are taking
■  Your credit card information or bank information for a pre-authorized payment plan
■ If you have or recently had group insurance, the name of the insurer, your group plan and subscriber ID, and the date the benefits ended
■  Your driver’s licence (if applying for catastrophic coverage)

For your added convenience, your affordable premiums can be paid semi-annually or annually by cheque, or monthly, semi-annually or annually by pre-authorized debit (automatic bank withdrawal) or credit card.
