Long Term Care Interview

What is a health interview?

As part of the underwriting process for Sun Long Term Care Insurance, a licensed nurse may interview you either by telephone or in person. Health interviews are used to gather additional information about your medical history and carry out cognitive exercises, such as word recall tests, that are required to underwrite your application. For a successful cognitive exercise it is important to follow the instructions of the interviewer in order to avoid a follow-up face-to-face interview.

Who performs the interview? 

Your interview will be conducted by a Canadian licensed nurse. The nurse is contracted by our service provider. LifePlans, to conduct the health interview. LifeRans conducts background checks and verifies the licenses of all nurses.

How is the interview scheduled? 

A LifePlans representative will call you to schedule an appointment at your convenience. The interview may take place at your home, place of business, or any other location that is convenient for you.
When the LifeRans representative calls, it may be displayed as a toll-free number. The representative will leave a message if you do not answer, so please call back to schedule your appointment to complete the interview.

What does the interview involve?

Due to the personal nature of the interview, please ensure that you can be in a location where you will be uninterrupted and comfortable answering these questions. During the interview, the nurse will ask you about things like:
Your employment, hobbies and activities
Your medical information and medications you take
Your memory
Your ability to perform everyday activities
While certain questions may have already been answered on the application, a licensed nurse will be confirming the answers and obtaining additional details when necessary.

Telephone Interviews 

Telephone interviews are intended for individuals age 60 and over, but may also be required for younger individuals, depending on information provided in the application. The telephone interview should take about 30 - 40 minutes to complete. Based on the information developed during this interview,a face-to-face interview be required.

Face-to-Face Interviews

Face-to-face interviews are intended for individuals age 70 and over but may also be required for younger individuals, depending on information provided in the application or during the telephone interview. Height, weight and blood pressure readings will be taken; however, disrobing, blood samples or the collection of other specimens will not be required. Depending on your histoiy, the face-to-face interview will take approximately one hour to complete. At the end of the interview,you will be asked to review, sign and date
the face-to-face interview form.
